腹腔镜保留脾脏的胰体尾切除术

自1996年Gagner等首次完成腹腔镜胰体尾切除术(laparoscopic distal pancreatectomy,LDP)以来,该术式得以不断推广[1]。由于胰腺的解剖位置紧邻脾动静脉及脾脏,传统观念将胰腺与脾认为同一解剖单位,从而将胰体尾联合脾脏切除术作为治疗胰体尾肿瘤的经典术式。近年来对脾脏功能的认识越来越深刻,精准外科中保留器官的理念不断深化,胰体尾切除术中是否联合脾脏切除这一问题得到重视。本文围绕腹腔镜保留脾脏的胰体尾切除术(laparoscopic spleen preserving distal pancreatectomy,LSPDP),将其理论基础及临床应用作一阐述和探讨。
