Comparison of clinical efficacy between surgery and observational treatment for adult hepatic hemangioma: a Meta-analysis

Abstract:

Objective

To systematically evaluate the clinical efficacy of surgery and observational treatment of adult hepatic hemangioma.

Methods

Relevant studies about surgery and observational treatment for adult hepatic hemangioma were searched in PubMed, EMbase, Ovid Medline, The Cochrane Library, Web of Science, China Biology Medicine disc (CBMdisc), CNKI, Wanfang database and Chongqing VIP database. The retrieval time started from the inception of databases to November 2019. The English and Chinese searching terms consisted of benign liver tumor, benign hepatic tumor, hemangioma, liver hemangioma, hepatic hemangioma, heamangioma, liver heamangioma, hepatic heamangioma, liver operation, liver surgery, liver resection, hepatic resection, hepatic surgery, enucleation, chemoembolization, radiofrequency ablation, medicine, conservative, observation and follow up. The included studies were subjected to analysis and quality assessment. Meta-analysis was performed with RevMan 5.3 software.

Results

26 relevant studies consisting of 2 908 cases were included, including 1 181 cases in the surgery group and 1 727 cases in the observation group. Meta-analysis revealed that the incidence of complications in the surgery group was significantly higher than that in the observation group (RR=9.61, 95%CI: 5.81-15.90,P<0.05). No significant difference was observed between two groups in the mortality rate (RR=1.36, 95%CI: 0.12-15.80,P>0.05) and the incidence of recurrence or aggravation of symptoms (RR=1.79, 95%CI: 0.35-9.07,P>0.05).

Conclusions

Compared with observational treatment, surgery increases the incidence of complications in patients with hepatic hemangioma. Surgical treatment should be adopted on the premise of strictly assessing the indications and timing of surgery.
