目的

探讨肝血管肉瘤的误诊原因并进行文献复习。

方法

回顾性分析2017年4月南昌大学第一附属医院收治的1例被误诊为肝腺瘤的肝血管肉瘤患者临床资料。患者女,47岁,因"体检发现右肝占位性病变1周"入院。患者已签署知情同意书,符合医学伦理学规定。上腹部CT和MRI平扫及增强检查均提示肝腺瘤并恶变可能,不排除肝脓肿等炎性病变。入院初步诊断为肝占位性病变,考虑为肝腺瘤可能性大。观察患者的诊断、治疗及疗效。

结果

患者于2017年7月行右半肝切除术。术后病理学检查结果为肝血管肉瘤。患者术后恢复顺利,术后8 d出院。

结论

肝血管肉瘤是一种罕见的间叶组织来源恶性肿瘤,临床表现和实验室检查均无特异性,影像学上与肝腺瘤、肝脓肿等难以鉴别,需行病理学检查确诊。

Objective

To explore the causes of hepatic angiosarcoma misdiagnosed as hepatic adenoma and review the relevant literature.

Methods

Clinical data of 1 patient with hepatic angiosarcoma who was misdiagnosed as hepatic adenoma in the First Affiliated Hospital of Nanchang University in April 2017 were analyzed retrospectively. The female patient, 47 years old, was hospitalized for the reason of a right hepatic lesion found by physical examination for 1 week. The informed consent of the patient was obtained and the local ethical committee approval was received. Plain and enhanced CT and MRI examination of the upper abdomen indicated a hepatic adenoma with malignant potential, while inflammatory lesion such as hepatic abscess was not excluded. The patient was initially diagnosed with hepatic space-occupying lesion after admission, and possibly a hepatic adenoma. Diagnosis, treatment and the efficacy of the patient were analyzed.

Results

The patient received right hemihepatectomy in July 2017. The postoperative pathological examination result indicated hepatic angiosarcoma. The patient recovered well after operation, and was discharged on the 8th day after operation.

Conclusions

Hepatic angiosarcoma is a rare malignant tumor derived from mesenchymal tissue. Its clinical manifestations and laboratory findings are non-typical. It is difficult to differentially diagnose hepatic angiosarcoma from hepatic adenoma, hepatic abscess with imaging findings, and pathological examination is needed for confirmation.
