目的

探讨Habib 4X射频止血切割器在肝切除术中的应用价值和安全性。

方法

回顾性分析2009年10月至2011年10月在昆明市第一人民医院因肝脏疾病接受肝切除术治疗的84例患者临床资料。其中男55例,女29例;年龄28~70岁,中位年龄46岁。患者均签署知情同意书,符合医学伦理学规定。所有患者均应用Habib 4X射频止血切割器进行肝切除术。手术方式:左半肝切除术16例,左外叶切除术18例,右半肝切除术12例,右肝单段、二段或者三段联合切除术18例,肝脏局部切除术20例。观察患者肝切除时间、术中出血量、围手术期并发症及死亡发生情况。

结果

84例患者应用Habib 4X射频切割器实施肝切除术均获成功,肝切除时间10~48 min,中位时间29 min;术中出血量100~900 ml,中位出血量320 ml。无发生围手术期死亡;患者无需接受外科重症监护,无发生大出血、肝衰竭、胆漏、腹腔感染、淋巴管漏等并发症者;发生腹腔包裹性积液40例,中等量及以上胸腔积液10例,经积极对症治疗后逐渐吸收痊愈。

结论

在肝切除术中应用Habib 4X射频止血切割器是一种安全、有效的方法。

Objective

To investigate the application value and safety of bipolar radiofrequency device Habib 4X in hepatic resection.

Methods

Clinical data of 84 patients received hepatic resection for liver diseases using Habib 4X at the First People’s Hospital of Kunming from November 2009 to November 2011 were studied retrospectively. There were 55 males and 29 females with age ranging from 28 to 70 years old and a median age of 46 years old. The informed consents of all patients were obtained and the ethical committee approval was received. All the patients received hepatic resection with the application of Habib 4X. Sixteen cases received left liver resection, 18 cases received left lateral lobectomy, 12 cases received right liver resection, 18 cases received combined resection of 1, 2 or 3 segments of right liver, and 20 cases received liver partial resection. The time of hepatectomy, the amount of introperative bleeding, perioperative complications and mortality were observed.

Results

All operations using Habib 4X were successfully performed in 84 cases. The time of liver resection was from 10 to 48 min with the median time of 29 min. The amount of bleeding in the operation was from 100 to 900 ml with the median amount of 320 ml. No perioperative death was observed and no case needed surgical intensive care and no complications such as massive hemorrhage, liver failure, bile leakage, abdominal infection, lymphatic leakage occurred. Encapsulated effusion was observed in 10 cases and moderate to high hydrothorax was observed in 10 cases and all were cured by effective symptomatic treatments.

Conclusion

It is safe and effective to apply Habib 4X in the liver resection.
